Notice to Former NH Serenity Place Patients

Concord, NH – Attorney General John M. Formella provides the following notice to Former Serenity Place Patients: In 2018, the National Council on Alcoholism and Drug Dependence of Greater Manchester d/b/a Serenity Place closed its doors. All patient files and records in the custody of Serenity Place at that time were transferred to the State of New Hampshire, Department of Health and Human Services, Division for Behavioral Health, Bureau of Drug and Alcohol Services ("BDAS") and are being held at BDAS 's office located at 105 Pleasant Street, Concord, NH 03301.

If you are a former patient and you want to obtain a copy of your own records, you can do so by completing a Request for Access to Client's Own Confidential Information Portable Document Format Symbol. If you are a person requesting a former patient's records with the patient's permission, you can do so by completing an Authorization Form Portable Document Format Symbol. Executed Requests and Authorizations must be returned to:

If you need a Request or Authorization sent to you by U.S. Mail, please call (p) 603-271-6113 to make that request and one will be mailed to you.

Once BDAS receives an executed Request or Authorization, a representative will contact you to schedule an appointment for you to pick up the records.

Please be advised that BDAS anticipates destroying Serenity Place patient files and records in its custody on October 1, 2022. You should, therefore, take prompt action to request a copy of your records.
